Background: history of the project

The current project has been developed during a mission in Sana'a in November 2005. It is the result of the analysis of working & functioning conditions in paediatric department and of discussions with the medical staff.

Visiting of the Professor Lequien Chief Of Neonatology Unit in Lille Hospital in France, Sanaa the 13.06.07 in As Sabeen Hospital Sanaa. Discussions about the cooperation between Lille Hospital and As Sabeen Hospital which will be signed on october 2007 -

  The context of the project

Al Sabeen hospital was built between 1988 and 1989. This is the only public hospital specialized in mother and child care. It is run by Sana'a city council and managed by the doctor Amat Alkareem Alhori.

It is a 337-bed hospital, divided in 19 department and 8 of them can accommodate infants and children.

In 2004, the hospital has accommodated 21735 patients, including 9697 childbirths, i.e. a daily average of 60 admissions and 26 deliveries. Among the 21735 patients, we can number 1447 babies were admitted in the nursery, 608 in the private department, 524 in the intensive care unit, 2551 in the paediatric emergency department and 1833 in the paediatric medicine department, i.e. a total of 6963 infants or children.

The number of out-patients is yearly 54016, daily 150.

Al Sabeen hospital employs 589 people, of which 126 doctors, 170 nurses (most of them from India), 128 people in biology and radiology departments; 78 people are assigned to cleaning & maintenance department; 87 people are assigned to management .

The chairwoman of LINK, Miss Nagatt Abdou, led two successive missions: one in 2004 with doctor GA. Loeuille, paediatrician at the Dunkirk hospital, and an other with doctor P.Lequien from the neonatal department of the Lille University Hospital in 2005. These two visits allowed the recognition of the needs of As sabeen Hospital and definition of objectives.

  Descriptive of the actual project

Objective of the project:

General purpose:

The main goal is optimization of local resources. The project of a post-surgical intensive care unit was supported by dynamic paediatric surgeon. It was rejected, considered as unrealistic according to the actual possibilities.

Specific objectives:

1- Set up rigorous hygiene measures in the neonatal department.

In all countries, the main danger for newborn babies admitted to hospital, is the “nosocomial infection" The prevention is mainly the hand washing.

This is a double-objective:

Installation of points for hand washing, which are missing at the moment.

Training of the nursing staff

These measures constitute an absolute priority. The expected result is an express and sensible diminution of morbidity and mortality of newborn infants

Create a neonatal « mother-child » unit.

Until now, there is no possibility to hospitalize lastingly a mother with her newborn baby. The consequence is the interruption of breast-feeding. Breast feeding was demonstrated in all the emerging countries as the best prevention of serious infectious diseases in the first year of life.

Here too, the gain in terms of morbidity and death rate is spectacular.

The architectural context of the hospital is perfectly suitable for the realization of this kind of project.

3- The rationalization of the emergency care department.

Emergency Department has an understanding popularity: its frequency shows its importance. Patients admitted in this department are, a mix of children with severe sickness, potentially transmissible, and of other patients with benign conditions.

Such promiscuity is obviously highly dangerous. Add to these contagious conditions, a rational patients screening become impossible.

A cautious organization might increase the efficiency of this structure without implying important means.

Contemplated actions:

Description:

Objective n°1 (organization of hand-washing)

Installation of washbasins at the entrance and inside the unit.

Installation of antiseptic-soap & paper towel dispensers

Suppression of ineffective and expensive measures like use of cover-shoes and single unit blouses.

Sensitization and training of the nursing staff to the respect of hygiene rules

Objective n°2 (creation of a neonatal « mother-child » unit)

Identification and fitting basic equipment : toilets and showers

Installation of a kitchen for mothers.

Acquisition of furniture: beds, tables and armchairs

Acquisition of breast-pumps

Training of the staff to assist the breast-feeding of a sick neonate.

Familiarization with breast-pumps

Objective n°3 (organization of casualty department)

The optimization of the department in relation with Yemeni professionals.

The improvement of the equipment in basic medical material: otoscope , ophthalmoscope, etc.

In cooperation with the physicians of the hospital, elaboration of protocols and procedures.

The implication of LINK  :

The feasibility of these projects is warranted by the local conditions : one site, Al Sabeen Hospital, and one partner, city council of Sanaa, and doctor Amat Al Kareem Al Houri, director. It excludes the implementation of expensive techniques.

It is remembered that Al Sabeen hospital is not supported by any other NGO.

Expected results

On quantitative way:

The expected benefits are more qualitative than quantitative. Nevertheless, these measures should decrease the morbidity and the mortality of children admitted to the Al Sabeen Hospital. The duration of hospitalization will be shortened thank prevention of nososcomial complications

On qualitative way:

It can be expected a better efficiency of the working conditions of the emergency unit.
